Hypoglycaemia and carb counter Topic

I am a T2 with sedentary lifestyle. Lately I have transformed myself into a moderately active person since this COVID situation became a threat to all diabetic T2. Putting a lot of effort to curb my carb intake the whole day but experiencing hypoglycaemia in the middle of the night and post workout.
Cannot take much risk with hypos at night so I forced to take some sweets to get my BG to normal. But as a result of that, the weight loss is not as expected. How do I handle this? I am on intermediary human insulin and have reduced the dose significantly. So I am getting spikes and hypos as if I am on a rollercoaster ride. Can’t use carb-insulin ratio as it’s not Bolus type. Feeling quite frustrated...

Hello Maddie,

First of all, congrats for becoming more active and taking charge of your health! I hope you soon prevent the low glucoses so you can continue on your healthy journey and improved diabetes management! Are you working closely with your doctor and/or diabetes educator? That is very important. They can help you tweak your insulin doses to your new activity levels so you are at less risk of lows. You may need to lower your insulin dose further and/or make sure you are including adequate healthy carbohydrate prior to a workout and as evening snack.

Are you on a very low carb diet or are you on a moderately low carb diet. You do need to eat adequate healthy carbs with insulin therapy. The trick is to choose healthier carb foods and to spread them throughout the day and balance the carbs with protein and fats.

Hi Brenda, thanks so much for taking time to respond to my question. Well, I have an appointment with my diabetic nurse in a week but not sure how much she will help. I am taking 180 diabetic carb count a day and following 45:20:35 balanced plan. Yesterday I took very little juice during workout which did not drop my BG but not sure if this is a right approach.
On top of that the most notorious “dawn effect” has started appearing for the first time in so many years of my diabetic journey..

I am yet to go through all the links you shared but the snacking one is already helping me.
Can you please let me know if 45:20:35 with 180 DCC is good for T2 or not? And is it okay to take juice during workout? Thanks a million for your time and response, it gives me so much assurance that someone is out there to help .. truly thankful.

Greetings Maddie-
I think Brenda's response, "make sure you are including adequate healthy carbohydrate prior to a workout and as evening snack" will really help prevent the low blood sugars you are having after a workout and in the middle of the night.
Before your workout, I would have some complex carbohydrates and protein. For example I would suggest a whole wheat cracker like Ak-Mak and natural peanut butter. Before bed I would suggest plain, greek yogurt and berries with a few toasted nuts.
-To answer your question, it is ok to have juice to treat a low blood sugar. Here's some more info: https://www.mayoclinic.org/diseases-conditions/hypoglycemia/diagnosis-treatment/drc-20373689
An "official" low is less than 70 mg/dl. If your blood sugar is below 70, yes! Please have juice or check out the other simple carbohydrates (above) to get your blood sugar back up. If your blood sugar is 100 and you want something quick because you are tired and feeling like you need a pickup and don't want your blood sugar to go lower, try 8 oz of fat free milk. We are so glad to hear you are taking your health seriously and getting in shape!
Also thrilled you have an appt with your diabetes team as they will be able to provide individualized care, that is best for you and your health.
You've got this! Joanna (MyNetDiary Dietitian)

Dear Joanna, all the ideas you and Brenda shared are amazing. I am going to implement them ASAP. My BG comes down to 4.2 ish which is 75 as per flash monitor so it’s just about that low BF level and I start feeling hypo like a weird type of sweating and some pressure in my head. I am going to have diabetes appo but not too much hopeful as the support is lesser than usual due to COVID. So I am counting on trial and error method using the knowledge coming from you all which is amazing. Thanks again for these lovely snacking ideas. Will be in touch and keep you posted. Thanks, Maddie
